Antonietta D’Angelo of Acer Osteopathic Clinic, based at 15 Ashley Green Road, Chesham, Bucks, HP5 3PE, which hereafter for the purposes of this Privacy Notice will be referred to as the Osteopath, is pleased to provide the following information:

Personal Data
To be able to process your personal data it is a condition of any treatment that you give your explicit consent to allow the Osteopath to document and process your personal medical data. For the purposes of providing treatment, the Osteopath may require detailed medical information about you, your baby or child. This includes your medical history, your symptoms, your medication and any relevant correspondence with other health care professionals. The information is vital for providing safe, appropriate care. This data is held securely, is not shared with anyone not involved in your treatment.
Bookings
The Osteopath uses your contact details provided by you – name, surname, postal address, phone number, email address – to make appointments, sends reminders and share information relevant to your care. This also includes administrative communications such as booking confirmation and pre-appointment forms.
Newsletter and Marketing
For marketing purposes, the Osteopaths may also use the contact details provided by you to respond to your enquiries, including making telephone contact and emailing information to you which the practice believes may be of interest to you. Newsletter sign-up is always opt-in but you can opt out from this at any time. The Osteopath uses Systeme.io. Data is stored securely according to GDPR.

When you fill in any forms: You may be asked to provide your personal details includes name, email, message, phone and company name in order to respond to your enquiry about the service that may be of interest or which you have requested – so that the Osteopath can verify your eligibility and respond appropriately. Any data stored is never passed on to third parties and our devices are password protected.
Burst Statistics:This website uses Burst Statistics, a Privacy-Friendly Statistics Tool to analyze visitor behaviour. For this functionality we collect anonymized data, stored locally without sharing it with other parties. For more information, please read the Privacy Statement from Burst. It does not use tracking cookies or share data with third parties, and visitors are not followed across the web, and the tool is built to comply with privacy regulations like GDPR.
Legal basis for processing any personal data
Contractual necessity: to carry out the osteopathic care and to manage your appointments.
Legitimate interest: to respond to enquiries concerning the services provided and to maintain records necessary for the running of the practice.
Explicit Consent: for the processing of your personal data for the purposes outlined and for marketing or newsletter communications you have opted into. You can withdraw consent at any time by using the postal, email address or telephone number provided at the end of this Privacy Notice.
How we share your data: the Osteopath will not sell, rent or share your personal data for commercial purposes. In some cases, it may necessary to share relevant medical information with other medical professionals such a GP, or another specialist which is in the interest of yourself or your child. In this case we will always inform you before doing so where possible. The Osteopath may also be required to disclose information to meet legal obligation.
Retention Policy: the Osteopath will process personal data during the duration of any treatment and will continue to store the personal data for eight years after the contract has expired to meet any legal obligations. After eight years all personal data will be deleted. Records concerning minors who have received treatment will be retained until the child has reached the age of 25.
Data storage and systems
Cliniko – is the practice software system used to keep medical records, appointments booking and patient communications. Cliniko’s servers are based in Australia, however, it is contractually committed to protecting patient data in accordance with UK GDPR.
Physitrack – used to assign home exercise and activity programmes. Data is stored securely in accordance to GDPR.
Systeme.io – used to manage newsletter sign-up. Data is securely stored according to GDPR.
Your rights under UK GDPR
At any point whilst the Osteopath is in possession of, or processing your personal data, all data subjects have the following rights:
Right of access – you have the right to request a copy of the information that we hold about you.
Right of rectification – you have a right to correct data that we hold about you that is inaccurate or incomplete.
Right to be forgotten – in certain circumstances you can ask for the data we hold about you to be erased from our records.
Right to restriction of processing – where certain conditions apply you have a right to restrict the processing.
Right of portability – you have the right to have the data we hold about you transferred to another organisation.
Right to object – you have the right to object to certain types of processing such as direct marketing.
To access what personal data is held, identification will be required
The Osteopath will accept the following forms of identification (ID) when information on your personal data is requested: a copy of your driving licence, passport, plus a recent utility bill or a bank statement not older than three months.
